Registering with the Practice
New patients registering with the practice will be asked to complete a registration form and Health Questionnaire. They are invited to attend a new patient medical with the practice nurse.
If you are registering a child, please bring details of which immunisations they have previously received.
If you are genuinely housebound and cannot attend to see the nurse then please let us know.
There will be a specified doctor taking on new patients at any one time (so that we keep our patient numbers fairly equal between us).
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
